A new α1-microglobulin test has been developed for the determination of this excellent marker for the evaluation of renal tubular function. α1-microglobulin is also elevated in heavy metal poisoning, following renal colic, decreased glomerular filtration and renal disease associated with diabetes. After its secretion into the blood, α1-microglobulin is found in free form and in a wide variety of complexes. Only the free form of α1-microglobulin is filtered through the glomerulus and then reabsorbed by the proximal tubule of the nephron where it is degraded. In conditions with impaired tubular function, α1-microglobulin reabsorption decreases and its concentration in urine increases.
